All players must register within their home boundaries,
being the City of Duncan, Municipality of North Cowichan,
CVRD Electoral Area E and G,Town of Ladysmith, Thetis Island,
Salt Spring Island (prefix 537) and CVRD Electoral Area H North to Takala Road.
All cheques payable to CVMHA and must be paid in full by August 1, 2010.
If interested in the Competitive Program,
including Atom, Peewee, Bantam and Midget there will be a
NON-REFUNDABLE $75.00 tryout fee due at the time of
registration, cheque dated June 1, 2010.
The 2nd Competitive fee for Atom is $350.00
due at the time of registration, cheque dated for Nov 1, 2010.
The 2nd Competitive fee for Peewee, Bantam and Midget is $350.00
due at the time of registration, cheque dated for Nov 1, 2010.
All registrations will require a Jersey deposit of $200.00 due at the time of registration,
cheque dated for March 31, 2011.
ALL CHEQUES MUST BE SEPARATE.
New Members may dowload the registration form.
Please bring with you a current hydro bill, players medical number and birth certificate.
*Please note there is a $20.00 administration fee for all new members.

Registrations received after this date will be subject to a additional charge
and be placed on a wait list.
